Please ensure that the following information is included in your referral:

Full client details including a family contact if needed
To-scale drawings and photos
MAC – referral code
NDIS – reference number, plan dates, who manages plan, and confirmation that the recommendation is in the clients budget (please note a home mods consultation fee will be applicable for major modifications)
Your contact details – please include your mobile or pager number (or another staff members details if you are part-time) in case the builder or installer needs to discuss any issues

We can arrange a joint visit with our builder to discuss your clients’ modification needs. Please contact us to schedule a suitable time – 9524 1100.
